RN - Case Manager

Must-Have:
Supports safe and timely patient discharge transitions, typically managing 14-18 patients.Coordinates durable medical equipment (DME) and post-acute services.Identifies high-risk patients and assesses discharge barriers.Collaborates with interdisciplinary team members.Documents and coordinates care in the electronic health record system.
Description:This role focuses on ensuring smooth transitions for patients being discharged.You will actively manage and coordinate the necessary services for patients' post-discharge needs.Effective identification of patients at risk and addressing barriers to discharge is critical.Collaboration with healthcare professionals is essential for optimal patient outcomes.Familiarity with electronic health record systems is beneficial for documentation and care coordination.
