The Care Team, in partnership with Traditions Health, is seeking a new Registered Nurse (RN) to join our growing Hospice Team in Cleveland.

Qualifications
Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Licensure: Current state license as an RN, current driver’s license.
Experience: One year of RN experience in a clinical care setting required; home‑health experience preferred.
Knowledge and Skills: Nursing skills as generally accepted standards; good interpersonal skills; CPR certification; ability to transport patients; ability to work flexible schedule and local travel.
Transportation: Reliable transportation and valid driver’s license and auto insurance.
Responsibilities
Administer skilled nursing care in patients’ homes for clients of all ages.
Coordinate care with the interdisciplinary team, patients, families, and referring agencies.
Complete initial and ongoing assessments, including OASIS or other appropriate tools.
Re‑evaluate patients’ nursing needs and outcomes, updating the plan of care as necessary.
Implement preventive and rehabilitative nursing procedures and initiate referrals to other services.
Prepare clinical and progress notes and complete documentation in a timely manner.
Supervise, teach, and provide clinical direction to home‑health aides, LPNs/LVNs, and paraprofessionals.
Perform on‑call duties and provide care to patients and families as assigned.
Communicate changes in patients’ condition to physicians and other team members.
Ensure compliance with third‑party payor requirements and maintain adherence to infection control measures.
Essential Functions
Coordinate and oversee patient care plans, discharge planning, and specialized nursing services.
Maintain documentation and monitor compliance with regulations and payor requirements.
Demonstrate professional growth through in‑service programs and competency improvement.
Promote the agency’s philosophy and administrative policies.
