Job description
Moments Hospice is seeking an RN Case Manager to coordinate patient care across our team and with patients' families. You’ll conduct initial assessments, refine care plans, and provide hands-on nursing care while keeping open lines of communication with the care team.
Qualified candidates are licensed RNs in their state, proficient with EMR systems, and able to manage a caseload with strong planning and organizational skills. A valid driver's license and reliable vehicle are required.
