Job description
Alternate Solutions Home Health Columbus is seeking a Registered Nurse Case Manager (RNCM) to lead assessment of patient condition and plan of care, provide case management and clinical care, and coordinate all disciplines under physician direction.
The RN educates patients and families, ensures patient safety, and maintains accurate documentation while staying compliant with regulations and the scope of practice. This role requires clinical leadership and collaborative teamwork.
