Job description
St. Vincent's Special Needs Center in Connecticut seeks an RN Case Manager to provide comprehensive nursing oversight across adult residential group homes for medically complex individuals.
You will coordinate care plans with interdisciplinary teams, ensure safety, manage health assessments, and support staff through training and audits. The role covers up to 3.5 homes with roughly 8–10 hours per home weekly in a collaborative coverage model.
