Job description
Sovah Health is seeking an RN Case Manager to coordinate and optimize patient care across our southern Virginia campuses. In this role, you will complete comprehensive assessments, develop and manage individualized care plans, and support safe, effective transitions of care. Working closely with physicians, nurses, social workers, and community partners, you will advocate for patients, ensure appropriate use of resources, and promote high-quality, patient-centered outcomes.
