RN Case Manager: Discharge & Care Coordination

Job description

Cynet Health is seeking an RN Case Manager for an 8-week assignment in New York. You will support safe patient discharge transitions, coordinate DME and post-acute services, and identify barriers to discharge while collaborating with the interdisciplinary team.

This role requires an active NYS RN license, 2 years of case management experience preferred, and relevant certifications. You will document care in the EHR system and help ensure timely patient outcomes.
