Job description
The Rector & Visitors of the University of Virginia in Culpeper, VA, is seeking an experienced RN Case Manager to join UVA Community Health. The RN Case Manager leads complex discharge planning, collaborates with physicians and the healthcare team, and coordinates care for medically complex patients.
Requirements include an Associate's Degree in Nursing (BSN preferred), current RN licensure, and 2+ years of healthcare experience with weekend/holiday coverage.
