Job description
The Valley Health System is hiring a Registered Nurse to lead discharge planning and utilization management efforts. You will work to ensure safe, individualized discharge and optimize reimbursement through proactive collaboration with patients, families, and the healthcare team.
The ideal candidate has 3+ years of clinical experience with knowledge of discharge planning, social determinants, and community resources, and will use computer-based tools for data collection and management.
