Job description
Advanced Health Care of Las Vegas is seeking an experienced RN Case Manager to assist the Director of Nursing in facilitating smooth and safe discharges for residents. This full-time role is weekdays with occasional weekends and holidays, requiring in-person collaboration across the care team.
You will coordinate discharge planning, set dates with IDT, prepare paperwork, arrange DME and transportation, provide resident education, and participate in quality assurance processes.
