Home Health (RN) Case Manager - Full Time
Due to expansion, and significant growth our Pediatric & Adult Skilled Division is seeking an aggressive candidate for a skies the limit opportunity. This position will be a salaried position paid weekly. Essential Job Functions/Responsibilities:
Receives referrals, determines services required, and Agency's ability to meet needs for pediatric and waiver patients.
Supervisory visits to observe clinical skills and ensure exemplary client care and compliance
Monitors documentation of pediatric and wavier patient clinical records to ensure compliance with regulatory standards of timeliness, accuracy, and completeness.
Manages/supervises a team of RNs, LPNs, Home Health Aides to provide effective and quality home care services
Qualifications: Prior experience in scheduling/human resources/customer service experience required.
Current license as a Registered Nurse in Pennsylvania.
Demonstrated organizational and time management skills, excellent communication skills, interpersonal skills, documentation skills critical thinking, and ability to work independently
Ambition to grow and advance with company goals.

Must be able to multi-task in a fast paced environment
