Job description
Bridge Home Health in Sonoma County is seeking a Registered Nurse Case Manager for full-time work in a home health setting. You will plan, organize, and direct home care services with emphasis on community health education and patient-family involvement.
Responsibilities include initial assessments, care planning, monitoring, and coordinating with physicians. Requires CA RN license, CPR, and 2+ years of licensed experience; valid CA driver's license preferred.
