Job description
Job Title: RN: Case Manager Address: Elizabethtown, NC
Job Description
Perform comprehensive assessments of high-risk members and develop individualized care plans.
Collaborate with primary care providers to implement targeted interventions and ensure appropriate care levels.
Identify and continuously reassess cost-effective and suitable levels of care for members.
Coordinate non-medical support such as housing and transportation to promote treatment adherence.
Manage care transitions by coordinating with physicians, social workers, discharge planners, and claims professionals.
Engage specialty resources as necessary to optimize health outcomes.
Document clinical, functional, and fiscal outcomes during treatment management.
Critical Information
Monday - Friday, 8:00 AM - 5:00 PM, on-call for PRN every 5th week.
On-site position in Elizabethtown, NC.
Compliance and submission requirements applicable.
Education/Licenses Needed
Associates or Bachelor’s Degree in Nursing or related field.
Current, unrestricted state Registered Nurse license.
Case Management certification.
