Home Health Care, Inc. is seeking an RN Case Manager for field-based home health in the Burnsville area. The role involves managing a defined caseload, performing OASIS assessments, and delivering skilled nursing care with patient education.
Expect collaboration with physicians and an interdisciplinary team in a supportive environment. The position offers a full-time schedule, with a Monday–Friday routine, one weekend per month, and one on-call shift per week.
