Job description
HCF of Warren Inc. is seeking an experienced Registered Nurse Case Manager to coordinate resident skilled care per physician orders and communicate with the interdisciplinary team, resident and family.
Responsibilities include on-site pre-admission assessments, admission processing, MDS coordination, QA participation, and discharge planning, with emphasis on maintaining professional competence and collaborating closely with the office team.
