The RN Case Manager - Float at Mass General Brigham plays a crucial role in enhancing patient care services along the continuum of care. This position is ideal for dedicated RNs who are passionate about improving patient experiences and outcomes.

What You'll Do
Collaborate with the healthcare team to enhance patient care services.
Continuously improve the patient experience and ensure high-quality care.
Evaluate, predict, and facilitate the trajectory of patient care.
Employ a collaborative process of assessment, planning, implementation, care coordination, monitoring, advocacy, and evaluation.
Participate in the Case Management department's orientation and onboarding program.
Engage in the \"Novice to Expert\" program for professional development.
Utilize resources from strong leaders and case management specialists.
Requirements
Education
ADN required, BSN strongly preferred
Experience
RN Case Manager experience required
Minimum of 2 years Med/Surg experience required
Acute hospital experience preferred
Licenses & Certifications
Must be a licensed RN in the state of Massachusetts
Case Manager certificate preferred
