Hinds LifeCare, one of the most well-known and well-respected nonprofit agencies in California’s Central Valley and a pioneer in the practice of hospice and palliative care, is seeking a Full Time RN Case Manager to join our team.The RN Case Manager is an experienced Registered Nurse who provides skilled palliative and end-of-life care in the community setting. As a member of the Interdisciplinary Team, the RN Case Manager facilitates the development of a collaborative interdisciplinary plan and ensures the overall coordination, collaboration and medical management of hospice services for patients and their significant others.The RN Case Manager is also responsible for promoting the philosophy of hospice, which redefines hope by focusing on quality of life and connection with loved ones during a patient’s final days, rather than on curing a disease.Why Hinds LifeCare:A mission-driven organization and a pioneer in hospice care, Hinds LifeCare seeks to uphold the dignity and ease the suffering of the terminally ill while supporting their loved ones and those who are grieving.
