Job description
WelbeHealth is hiring a Case Manager, RN in Elk Grove, CA to guide participant care within the interdisciplinary team. You will conduct nursing assessments, develop and monitor personalized care plans, and support transitions of care coordination to ensure safe, effective, person-centered outcomes.
Ideal candidates hold a California RN license, BSN preferred, current BLS/First Aid, and at least 2 years of nursing experience with frail/elderly populations.
