The Waters is seeking an RN Case Manager to support the Director of Health and Wellbeing and oversee nursing processes to deliver exceptional, resident-centered care in our community. You will serve as a primary contact for residents, families, and providers, ensuring courteous, dignified service.
Responsibilities include assessing care needs, collaborating with interdisciplinary teams, and maintaining accurate clinical documentation while promptly addressing emergencies and safety concerns.
