About the Role:
ComForCare Home Health Care - Florida Tampa/NW Tampa is looking for a dedicated and compassionate RN Case Manager to join our growing team in Tampa/NW Tampa, FL. In this assessment-focused role, you'll make a meaningful difference in patients' lives by conducting in-home evaluations and ensuring they receive the highest quality of care. Responsibilities:
Conduct comprehensive in-home patient assessments to determine care needs and eligibility
Develop and document individualized care plans based on assessment findings
Evaluate patient health status, functional abilities, and home environment safety
Collaborate with physicians, care teams, and family members to coordinate appropriate services
Ensure accurate and timely clinical documentation in compliance with regulatory standards
Monitor and reassess patient conditions, adjusting care plans as needed
Serve as a patient advocate, connecting clients with community resources and support services
Requirements:
Active and unrestricted Registered Nurse (RN) license in the state of Florida
Prior experience in home health, case management, or community health nursing preferred
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal skills with patients and families
Ability to work independently and manage a flexible schedule in the field
Reliable transportation and valid driver's license required
Proficiency with electronic health records (EHR) and clinical documentation
