What You’ll Do (High Level)
Build and guide care plans that keep clients safe and supported at home
Partner with families, physicians, and care teams to keep everyone aligned
Support and coach field staff so they feel confident and cared for
Spot problems early and help solve them before they become crises
Help ensure care stays consistent, covered, and well-coordinated over time
Keep documentation clear and timely—so care is protected and benefits aren’t delayed

What You Need
Active RN license
A steady, compassionate leadership style
Strong communication and follow-through
Comfort guiding others and building trust with families
Home health experience is a plus (but not required for the right nurse)
