Job description
Rochester Regional Health Home Care in Rochester, NY is seeking an RN Case Manager to deliver comprehensive nursing care to assigned patients, assess needs, develop and evaluate a Plan of Care, and coordinate interdisciplinary services.
This full-time role involves documenting care in an electronic medical record, supervising PDN as appropriate, coordinating with the care team, and traveling to patient sites within the service area.
