Job description
Hcr Home Care in Rochester, NY is seeking a qualified RN Case Manager for home visit assignments in Wayne County. You will provide skilled nursing care, follow physician orders, and lead a care team to ensure high-quality patient outcomes.
Responsibilities include patient assessment, care planning, supervising Home Health Aides, and coordinating with families and physicians. A NYS RN license and at least one year of home care experience are required.
