Job description
Amedisys is seeking a Registered Nurse Case Manager to provide in-home care coordination in the Tri‑Cities area (Petersburg, Colonial Heights, Hopewell, Chester, Prince George). This full-time day role offers competitive compensation and a point pay system after orientation.
You will assess patients, develop and implement plans of care, supervise LPNs and HHAs, and maintain thorough documentation while collaborating with the care team to improve patient outcomes at home.
