What You\'ll Do:
Conduct comprehensive patient assessments and develop individualized care plans.
Coordinate with interdisciplinary teams to ensure seamless, high-quality care.
Educate patients and families on managing health at home.
Document care with accuracy, compassion, and compliance in mind.
Schedule: Full-time, day shift
Compensation: Competitive salary based on experience
Bonuses: Relocation and sign-on bonuses available (case-by-case)
Employment Type: W2, direct hire with our client
What You\'ll Bring:
Active RN license in Idaho (or compact state)
At least 1 year of home health experience
Strong communication and organizational skills
A passion for patient-centered care
