Job description
Amedisys is seeking an experienced RN case manager to join our home health team in Woburn, MA. This full-time day shift role offers meaningful patient care and opportunities to collaborate with a multidisciplinary care team.
You will perform patient assessments, develop care plans, coordinate referrals, and provide education to patients and families. A valid RN license and CPR are required, with a sign-on bonus of $10,000.
