Job description
Amedisys is seeking an RN case manager for full-time days in Chester and Montgomery Counties, PA, with a $5,000 sign-on bonus and a salary range of $90,000.00-$93,000.00 annually. Join a leading home health and hospice provider delivering compassionate care in the patient's home.
Responsibilities include patient assessments, care plan development with the care team, referrals as needed, and maintaining progress records. On-call duties and other team responsibilities are required.
