Job description
Angels Care Hospice in Flagstaff, AZ is seeking a Registered Nurse Case Manager to coordinate all aspects of a patient’s home care and provide professional nursing care.
You’ll perform admission visits, determine eligibility for home care services, develop plans of care, and collaborate with other medical disciplines. Requires RN license, CPR, valid driver’s license, and 1 year experience.
