Job description
Terrace Home Health and Hospice in Boise–Meridian, ID is seeking a full-time RN Case Manager to coordinate patient care during episodes. You will visit patients, collaborate with physicians, and ensure timely data like OASIS and lab values are up to date.
You must hold an Idaho RN license, CPR certification, and have clinical experience. Strong communication, professionalism, and problem-solving are essential to manage diverse settings and maintain high-quality care.
