Job description
HCR Home Care in New York seeks an RN Case Manager to coordinate skilled nursing services, manage a full caseload, and ensure care plans align with physician orders.
You will supervise and teach family members and other nursing staff, conduct admissions, evaluate patient needs, and maintain compliant documentation.
This field-based role requires NYS RN license, at least one year of home care experience, and strong communication and organizational skills.
