RN Case Manager – Home Health (Jackson / Ann Arbor / Ypsilanti Territory)
Job Location: Farmington Hills, MI 48331
Job Types: Full-Time, Per Diem, Contract, PRN
Work Location: Field-Based (Jackson / Washtenaw Corridor)

Responsibilities
Conduct comprehensive nursing assessments
Develop individualized plans of care
Deliver skilled nursing interventions in the home
Educate patients and families on condition management and medications
Coordinate with physicians, therapists, and interdisciplinary team members
Complete accurate and timely documentation in WellSky / Kinnser
Utilize Home Health Notify for real‑time communication and updates
Ensure compliance with state, federal, CHAP, and Joint Commission standards

Qualifications
Graduated from an accredited school of nursing
Hold an active Michigan RN license
Have at least 2 years of nursing experienceHome health experience preferred
Are compassionate, organized, and resourceful
Have strong assessment and communication skills
Have reliable transportation, valid auto insurance, and a driver’s license
