Job description
Niagara Hospice Inc. is seeking an RN Case Manager to join our home care team in Lockport, NY. The role provides total, comprehensive patient care and oversight of the plan of care, with a Monday–Friday schedule and weekend rotation every 8 weeks.
Compensation includes benefits such as PTO, holidays, health insurance with employer-paid options, and retirement plans. The position requires NYS RN licensure and a valid NYS driver's license, with 3 years of nursing experience preferred.
