Position Overview:
We are seeking a Field RN Case Manager for a client in Bethlehem. This role involves providing direct patient care and managing cases within the community to improve healthcare outcomes. The position offers competitive salary, on-call stipends, and mileage reimbursement. Final hiring decisions are made by our client.
Responsibilities:
Assess patient conditions and develop care plans.
Maintain medical records and report patient status.
Serve as the primary contact for patients, families, and healthcare providers.
Educate patients and families on health management and prevention.
Coordinate with healthcare professionals for continuous care.
Requirements:
Active RN license in Florida without restrictions.
Minimum of 1 year supporting in-home patients.
Proficiency in case management software, strong organizational and communication skills.
Empathetic, detail-oriented, and able to work independently in the field.
