Responsibilities
Assess patient conditions and develop appropriate care plans.
Maintain comprehensive medical records and report patient status to necessary parties.
Act as the primary point of contact among patients, their families, and healthcare providers.
Educate patients and families on health management and preventive measures.
Coordinate with healthcare professionals to ensure continuous care and efficient case management.
Requirements
License: Active RN license in Arizona with no restrictions.
Experience: At least 1 year in a similar in-home healthcare role.
