Job description
Pathways Home Health & Hospice is seeking a Registered Nurse Case Manager to deliver skilled nursing care in patients' homes and coordinate with the clinical team across the Cupertino–Redwood City territory. This full-time role is based in Sunnyvale with M-F hours and rotating weekends.
You will perform comprehensive assessments, develop and adjust plans of care, supervise Home Health Aides, document care, and collaborate with physicians and team members.
