Job description
The Christ Hospital Home Health Care in Cincinnati, OH is seeking a Registered Nurse Case Manager (RNCM) to lead patient assessments, develop and coordinate plans of care, and oversee clinical care across disciplines under physician direction. You will educate patients and families, ensure safety and quality, document accurately, and stay compliant with regulations.
One year RN acute care experience preferred; home care experience desired.
