Job description
Jewish Family Service Association of Cleveland is seeking a full-time Registered Nurse Case Manager to deliver comprehensive nursing care in patients' homes. The RN will assess, document, educate, and coordinate with physicians and team members to improve outcomes.
Responsibilities include completing OASIS and case documentation, performing nursing procedures, and traveling across the service area with on-call duties to ensure 24-hour coverage. Ohio RN licensure and in-home experience required.
