Job description
Angels Care Hospice in Jerome, AZ is seeking a Registered Nurse Case Manager to coordinate all aspects of the patient’s home care. You will interact with other medical disciplines as needed and provide professional nursing care, including performing the admission visit and determining eligibility for home care services.
