nPosition Overview:
nAn experienced RN who is responsible for the overall management of patients, including the provision of direct care with a designated visit productivity standard. This position plans, organizes, and directs all patient care services for patients in the assigned caseload in accordance with current standards and regulations, ensuring the optimal degree of quality care is maintained. Responsible for coordinating the interdisciplinary team to implement the established plan of patient care and ensuring effective management of visit utilization and control of expenses. May assist in agency wide orientation, competency evaluations and consultation to clinical staff related to specific expertise.

nnBLS-Basic Life Support Healthcare Provider

n nTYPICAL EXPERIENCE:
nn2 years recent relevant experience
n nSKILLS AND KNOWLEDGE:
nnnMust have the ability to function independently and as a member of an interdisciplinary team; ability to exercise independent, sound nursing judgment in planning and providing patient care.
n
nnExhibits knowledge of patho-physiology and accepted treatment protocols for common home care diagnoses.
n
nnKnowledge of basic safety and infection control principles.
n
nnDemonstrated clinical assessment skills.
n
nnAbility to recognize the special needs of Hospice patients and others in the home.
n
nnAbility to communicate with patients and significant others in an effective, mature, caring manner.
n
nnAbility to maintain harmonious constructive relationships with internal and external customers.
n
nnDemonstrated effective written and verbal communication skills.
n
nnUnderstanding of palliative care for the terminally ill patient.
n
nnMust be able to recognize unsafe or emergency situations and act appropriately.
n
nnMust be able to meet company productivity standards as indicated by organization and manager.
n
nnObjectively and effectively solves unique problems as they arise or identify when to consult supervisor.
n
nnMust be able to deal with challenging work environment with time demands and occasional conflicting priorities.

nPay Range is $71.18 to $99.65 / hour
nThe compensation range may vary based on the geographic location where the position is filled. Total compensation considers multiple factors, including, but not limited to a candidate's experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs. Base pay is only one component of Sutter Health's comprehensive total rewards program. Eligible positions also include a comprehensive benefits package.
