Job description
St. Croix Hospice is seeking a Registered Nurse Case Manager to provide 1:1 home-based, whole-person care for patients and families.
You will coordinate individualized care plans with an integrated care team, supporting families through complex decisions, and guiding symptom management with empathy. The role requires an RN with a nursing degree and 1+ years of acute care experience; hospice/home health experience is preferred.
