Job description
St. Croix Hospice in Omaha, Nebraska is seeking a Registered Nurse Case Manager to provide 1:1, whole-person care to patients and families in their homes.
You will coordinate individualized care plans with an integrated care team and support families through complex decisions with empathy and professionalism. Requirements include an RN from an accredited program (AS/BS/MS), 1+ year acute care experience (hospice/home health preferred).
