Job description
VHS Hospice, proudly supported by Marquis Health Consulting Services, is seeking an RN Case Manager in Newport News and surrounding VA locations. This full-time role focuses on care coordination, clinical decision support, and ensuring continuity of care across the healthcare continuum.
You will conduct comprehensive assessments, develop individualized care plans, monitor progress, and collaborate with interdisciplinary teams.
