Pay
$90,000 – $130,000 DOE

Greater Twin Cities Metro Area, 21‑County Service Area (territories assigned based on coverage needs)

What the Role Involves
Support patients through complex physical and emotional transitions.
Provide clear, compassionate education and reassurance to families.
Coordinate a seamless end‑of‑life care experience from admission forward.
Manage pain, symptoms, and comfort‑focused care plans.
Collaborate with social workers, aides, chaplains, and medical directors.
Maintain accurate, timely documentation to support high‑quality care.
We’re Looking For Someone Who
Holds an active Minnesota Registered Nurse license.
Has experience in hospice, oncology, or palliative care.
Balances clinical skill with heart‑centered care.
Communicates with empathy, clarity, and confidence.
Understands the sacred nature of end‑of‑life care and approaches it with grace.
Veteran Friendly Employer
