nResponsibilities

nAt the direction of the Clinical Manager, coordinates/manages a case load comprised of Hospice patients. Assesses, plans, implements, coordinates, and evaluates care for each patient/family in conjunction with the hospice interdisciplinary team. Case management includes but is not limited to; comprehensive needs assessments, development of appropriate plan of care, referral to internal and external resources, evaluation of outcomes, and provision of nursing educational, clinical and related services. Supervises the Licensed Nursing Assistants/ Home Health Aides.

nnnProvides a full range of professional nursing care to patients in their homes under the medical direction of a licensed physician.

n

nnRegularly reevaluates patient's nursing needs and informs physician and other personnel of any changes in patient's condition or needs.

n

nnInitiates the plan of treatment and any necessary revisions.

n

nnSupervises the Home Health Aides. Teaches nursing care to be given in the home and reviews and explains the patient care plans.

n

nnPerforms initial and subsequent assessments of patient/family physical, emotional, psychosocial, spiritual and cultural needs to ensure and coordinate the delivery of comprehensive, comfort-focused services through the development of the Hospice patient plan of care.

n

nnRepresents VNH in a positive, proactive manner while participating in community education/support/ PR events.

n

nnMaintains continuity of patient care by planning and exchanging information with IDT members, Department personnel and other health and social agencies as is appropriate.

n

nQualifications

nREQUIRED QUALIFICATIONS

nnnMust be a licensed Registered Nurse in Vermont and New Hampshire or hold a compact license.

n

nnGraduate of an accredited school of nursing

n

n nPREFERRED QUALIFICATIONS

nnnMinimum two years’ general experience in a supervised nursing setting recommended.

n

nnHospice/Homecare experience preferred.

n

nnCertified in Hospice/Palliative Care Nursing desirable.

n

n nRequired Licensure/Certifications

nnnMust hold at least one active unencumbered RN license in either unencumbered VT or NH at the time of hire. Employees must have active unencumbered RN licenses in both VT and NH within 30 days of their hire date.

n

nnValid driver's license with a clean driving record

n

nnCurrent car insurance, which meets minimum standards

n

nnBLS within 90 days of hire

n

n nnnArea of Interest:Nursing

n

nnPay Range:$85,280.00/Yr. - $120,120.00/Yr. (Based on 40 hours per week, otherwise pro rata)

n

nnFTE/Hours per pay period:1.00 - 1.00 - 40 hrs/week
