Job description
St. Croix Hospice is seeking a Registered Nurse Case Manager to provide 1:1, whole-person home care for patients and families, coordinating with an integrated care team to build and adjust care plans, and supporting families through complex decisions with empathy and professionalism.
The role requires an RN with 1+ years of acute care experience (hospice/home health preferred). Travel to patient homes is part of the job, with a schedule from 8:00am to 5:00pm and on-call responsibilities.
