Travel RN - Hospice Case Manager Opportunity | Sunnyvale, CA
Join a compassionate healthcare team as a Hospice RN Case Manager in beautiful Sunnyvale, California!
Location: Sunnyvale, CA
Contract Length: 13 Weeks
Schedule: Days, 9:30 AM - 6:00 PM
Schedule may be flexible based on patient availability
Weekend Requirement: One weekend per month
Guaranteed Hours: 40 Hours/Week
Job Responsibilities:
• Admit patients to hospice services and complete comprehensive assessments
• Evaluate patients' physical, psychosocial, environmental, safety, and developmental needs
• Develop and implement individualized care plans
• Coordinate care with an interdisciplinary hospice team
• Document patient observations, interventions, and progress
• Participate in quality improvement initiatives and provide follow-up visits as needed
Coverage Territory: Millbrae, Burlingame, San Mateo, Foster City, Belmont, Redwood City, and surrounding areas.
Requirements:
• Active RN License
• Minimum 1 year of Hospice experience
• Current BLS/BCLS Certification
