Job description
TEEMA is seeking a compassionate RN Case Manager to coordinate care for elderly patients in a hybrid clinic and field setting in California. You will partner with PCPs and an interdisciplinary team to assess, plan, and implement care across the patient journey.
The role emphasizes reducing hospitalizations, educating families, and ensuring high-quality, patient-centered outcomes with a Monday–Friday schedule and a 13-week assignment.
