Job description
21st Century Home Health Services (21HHS) is seeking an experienced Registered Nurse Case Manager, Level 2, to lead patient care in Yuba City and surrounding areas.
You will create, implement and manage comprehensive plans of care, supervise two nurses, perform start of care, evaluations, and discharge planning, and collaborate with an interdisciplinary team to promote high-quality in-home care and reduce hospitalizations.
