Job description
UCLA Health is seeking an experienced RN Case Manager for the swing shift in Los Angeles. You will assess and coordinate care for a diverse patient population, collaborate with a multidisciplinary team, and develop individualized care plans using evidence-based risk stratification to ensure safe, high-quality, cost-effective care.
