Job description
Midland Care Connection, a nonprofit senior care provider in northeast Kansas, is seeking an RN Case Manager to coordinate comprehensive home-based care for older adults and seriously ill individuals in the Topeka area.
You will assess health status, develop individualized care plans, and work with an interdisciplinary team to ensure continuous, high-quality care aligned with PACE standards and the organization’s mission to promote independence and dignity.
